THERAPEUTIC REHABILITATION PROGRAM:

TRP is a mental health therapeutic rehabilitation program that focuses on building social, personal, and daily living skills, as well as the restoration of these skills. Our focus is to provide a safe environment for individuals to reduce the impact of mental health on their overall functioning so that they are able to function at their best, within their home, community, school and job setting. This program is eligible for medicaid eligible participants.

  • I-ASPIRE for girls ages 12-17

  •  NEW Perspective: Boys & girls ages 12-17

COMMUNITY BASED SERVICES:

 

Targeted Case Management

Our targeted case-management (TCM) provides community based services that assists clients with access to resources needed for housing, education, transportation, etc. We collaborate with community partners and agencies, to ensure that all the clients needs are being met. This program is eligible for medicaid eligible participants

Community Support Services

Our community support associate (CSA) assist with supporting clients with their daily living skills such as housekeeping, cleaning, budgeting, etc. The CSA provides therapeutic support to clients by supporting them in the implementation and attainment of their goals, outlined in their individual treatment plan in their home and community. This program is eligible for medicaid eligible participants.
